Voluntary Benefits
DrinkPAK provides all active employees with the option of purchasing additional Life and AD&D insurance for yourself, spouse, and/or child(ren) through MetLife at low group rates! When you enroll yourself and your dependents in this benefit, you pay the full cost through post-tax payroll deductions. Please note that you may need to complete an evidence of insurability form if you elect an amount above the guaranteed issue. Current participants may increase their election by one increment level during Open Enrollment, up to the Guarantee Issue amounts, without providing proof of good heath. Guarantee issue amounts are listed below.

The coverage amount for your Spouse cannot exceed 50% of your additional Life Coverage.
Child(ren) under 15 days old: Benefit limited to $100 of coverage. Child(ren) 15 days to 6 months old: $500. Child(ren) 6 months old to 26 years old, see above table for options.
Rates for your spouse are based on your age; please refer to Workday for rates.
