Eligibility
Eligible Dependents
You are eligible to participate in the DrinkPAK Program if you:
- Are a full-time employee scheduled to work a minimum of 30 hours per week
- Have satisfied the new hire waiting period of First of the Month following Date of Hire
You may also elect coverage for your:
- Spouse or Domestic Partner
- Dependent children (refer to the carrier’s contract for age limitations and/or requirements)
- Unmarried children who are physically or mentally incapable of self-support
Documentation Requirements for Dependents
If you enroll your spouse, domestic partner, and/or eligible dependent child(ren), when asked, you will need to provide documentation (noted below) to DrinkPak HR Department to confirm their eligibility for coverage.
- Marriage license for spouse
- Domestic partner attestation
- Birth certificate for children
- First page of your most recent tax return, listing eligible dependents (for spouse and/or children)
- Court-ordered guardianship papers, adoption papers, or placement letter
- Divorce decree to show parent/child relationship when names don’t match or to identify responsibility for providing health coverage
- For Domestic Partners: proof of common residence and documentation of mutual obligation (e.g., mortgage, rental agreement, loan, or some other co-debt)
